A New Hampshire medical power of attorney form, or advance directive form, is a dual-purpose document consisting of a durable power of attorney for healthcare and a living will. A person can choose to execute both sections of the advance directive, or they may choose only to complete one or the other. The durable power of attorney portion allows the principal to appoint an attorney-in-fact who will make important healthcare decisions, along with the advice of medical professionals, in the event of the principal’s incapacitation. This appointed agent can be a spouse, relative, or close friend, ideally someone who shares the principal’s values and who will always act with their best interests in mind. The living will section of the document pertains to life-prolonging treatment and healthcare options. The principal can use this part of the document to select the types of treatments they wish to receive (or avoid, as the case may be) in the event of an incurable injury, disease, or illness.
