A New Jersey minor guardianship power of attorney form, when executed properly, designates an eligible individual as a temporary guardian over a person’s child or children. The appointed guardian will handle all responsibilities and tasks associated with parenting such as educational matters, healthcare decisions, disciplinary actions, and other key components of raising a child. A guardianship power of attorney is often established in situations where a parent knows they won’t be at home for a significant period of time. However, it’s important to note that another power of attorney document will have to be created after six months if the parent wishes the guardianship arrangement to continue.

Signing Requirements (§ 3B:12-74) – Two witnesses.
