The Kentucky minor power of attorney form permits a parent to designate an agent to have health care and educational decision making powers on behalf of a minor child. This agreement is temporary and is usually employed for periods during which the parent will not be present due to work, education, or illness. The form is required to be signed in the presence of a notary public, who can be found at any branch bank location.
