The Kentucky minor power of attorney form permits a parent to designate an agent to make health care and educational decisions on behalf of their child. This agreement is temporary and is usually established for periods during which the parent will not be present due to work, education, illness, military service, or other reasons. While Kentucky statutes do not specify how long a guardianship arrangement may last, it is common for a minor power of attorney to remain in effect for six (6) to twelve (12) months. This document can only be revoked by the legal parent/guardian who authorized it.
Laws – § 27A.095
Signing Requirements – Not specifically mentioned in State law but the form does require that a notary public acknowledge the signatures inscribed on the document.
