If the principal cannot present themselves to the offices of an Ohio Bureau of Motor Vehicles, this form will allow the attorney-in-fact to appear in their stead and sign any papers associated with the titling or registration of a particular vehicle. The complete names and addresses of the principal and attorney-in-fact must be entered as well as all the necessary vehicle information. Once complete, the form can be taken to a notary public for acknowledgment.
