Medical Power of Attorney Colorado Form – Living Will – PDF
Use when a person may not be able to take care of their own medical decisions in the future so that he or she can select someone else to make them. It is common for a person to elect someone else if he or she may be going into surgery or in the early stages of a mental disorder.
In Colorado, through the Declaration as to Medical or Surgical Treatment, more commonly known as the Living Will, you can declare what types medical services you would or would not like to receive, and under what circumstances. Through the Medical Durable Power of Attorney, you can name an Agent to direct your health care when you no longer can. The absence of these documents takes the direction of your care out of your hands.
