A Hawaii medical power of attorney form consists of two (2) documents: the living will and the durable power of attorney for health care. Through the living will, a resident can prescribe not only what type of life-extending health care procedures they’d like to receive, but also under which circumstances they’d reject or accept them. With the durable power of attorney for the health care portion, the individual would be able to select a healthcare agent, an individual with the legal authority to make health care decisions on the patient’s behalf when they cannot make those decisions themselves.
