A Louisiana minor power of attorney form is a document that allows a parent to temporarily grant powers to another individual to act as the child’s guardian. In Louisiana, the more common term for a minor power of attorney is “provisional custody by mandate.” Custody of one’s children should only be given to a trusted individual, someone who is responsible enough to make good choices for the child. The parent can choose which powers, or “provisions,” the guardian will be able to exercise. This form is valid for one year after its execution unless the parent chooses to revoke it before the year is up.

Laws

  • Statute – § 951, 952, 953, and 954 (Provisional Custody by Mandate of Persons Having Parental Authority)
